Upload a diagram/sketch of the outdoor dining area requested to allow dogs. The diagram should be accurate and to scale, but does not need to be prepared by a licensed design professional. The following information should be included on the diagram:
Specify the days and hours of operation dogs will be permitted.
Prior to the permit being issued, a $50.00 application fee must be mailed or paid in person to the address below:
